Respostas clínicas e laboratoriais à perda sanguínea aguda e transfusão homóloga em asininos

| dc.description.abstract | Objetivou-se avaliar as respostas clínicas, hematológicas, hemogasométricas e bioquímicas de asininos submetidos à perda sanguínea aguda e transfundidos com sangue total homólogo. Doze asininos, machos, adultos, foram divididos em dois grupos de seis, onde cada grupo foi realizado um estudo distinto. No primeiro estudo (Grupo Perda – GP), os animais foram submetidos a uma perda sanguínea de 30% e avaliados nos tempos: T0 (antes da perda), T1, T6, T12, T24, T48, T72 (horas), e T8d, T16d, T24d e T32d (dias) após a perda sanguínea. No segundo estudo (Grupo Transfusão – GP), os animais foram submetidos à mesma perda sanguínea e após o período de 6 horas transfundidos com sangue total homólogo (20ml/kg), sendo avaliados nos tempos: T0 (antes da perda), 6 horas após perda e antes da transfusão (T6) e T1T, T6T; T12T, T24T; T48T; T72T (horas), e T8d; T16d, T24d e T32d (dias) após a transfusão de sangue total homólogo. O GP apresentou elevação da FC e FR depois de retirada do sangue retornando ao nível basal em T12 horas. O TPC teve aumento em T1 retornando ao fisiológico em T6. O hematócrito, hemoglobina e hemácias reduziram seus valores em T1 retornando para o nível basal em T16d para hematócrito e hemoglobina, e em T32d para hemácias. As variáveis hemogasométricas não sofreram variação após a perda sanguínea. O lactato e a glicose tiveram elevações em T1, tendo o lactato restabelecido em T6 e a glicose em T12. A proteína total diminuiu em T1 restabelecendo em T16d A Albumina, GGT, AST, ureia e creatinina não tiveram alteração durante o estudo. No estudo da transfusão (GT) todos os animais se mostraram compatíveis pelo teste de reação cruzada, sendo a transfusão realizada sem ocorrência de reações graves. Observou-se taquicardia, taquipnéia e aumento do TPC após a remoção do sangue sendo essas alterações compensadas na primeira hora após a transfusão (T1t). Os dados hematológicos após redução em T6 retornaram aos valores basais em T6t. A avaliação hemogasométrica mostrou estabilidade após a hemorragia e a transfusão. A proteína total teve restabelecimento 1 hora após a transfusão (T1t) e a glicose em T6t. A albumina apesar da perda sanguínea e transfusão se manteve sem alteração em relação ao T0. As variáveis renais, ureia e creatinina, se mantiveram normais e constantes e o GGT variou dentro da referência para a espécie. Assim, pode-se concluir que a perda aguda de 30 % da volemia é um evento patológico reconhecidamente grave, causando alterações clínicas, hematológicas e bioquímicas em asininos. Porém, os asininos do GP se mostraram capazes de reverter em até 12 horas as alterações nas variáveis clínicas e bioquímicas, a exceção da PT, e em até 24 dias as hematológicas, sem transfusão. Por sua vez, os animais do GT tiveram parâmetros clínicos, hematológicos e bioquímicos restabelecidos em até 6 horas e não houve lesão hepática ou renal após a terapia sanguínea, dessa forma é possível concluir que transfusão homóloga em asininos é uma terapia segura e aplicável na rotina clínica de asininos. | |
| dc.description.abstract2 | This work aimed to evaluate the clinical, hematological, blood gas and biochemical responses of donkeys subjected to acute blood loss and transfused with homologous whole blood. Twelve adult male donkeys were divided into two groups of six, where a separate study was carried out in each group. In the first study (Loss Group – GP), the animals were subjected to a blood loss of 30% and evaluated at the following times: T0 (before the loss), T1, T6, T12, T24, T48, T72 (hours), and T8d, T16d, T24d and T32d (days) after blood loss. In the second study (Transfusion Group – GP), the animals were subjected to the same blood loss and after a period of 6 hours transfused with homologous whole blood (20ml/kg), being evaluated at the following times: T0 (before loss), 6 hours after loss and before transfusion (T6) and T1T, T6T; T12T, T24T; T48T; T72T (hours), and T8d; T16d, T24d and T32d (days) after homologous whole blood transfusion. The GP showed an increase in HR and RR after blood withdrawal, returning to baseline levels at T12 hours. TPC increased at T1, returning to physiological levels at T6. Hematocrit, hemoglobin and red blood cells reduced their values at T1, returning to the baseline level at T16D for hematocrit and hemoglobin, and at T32d for red blood cells. Hemogasometric variables did not change after blood loss. Lactate and glucose increased at T1, with lactate reestablished at T6 and glucose at T12. Total protein decreased at T1 and reestablished at T16D. Albumin, GGT, AST, urea and creatinine did not change during the study. In the transfusion study (GT), all animals were compatible by the cross-reaction test, and the transfusion was carried out without the occurrence of serious reactions. Tachycardia, tachypnea and an increase in TPC were observed after blood removal, with these changes being compensated in the first hour after transfusion (T1t). Hematological data after reduction at T6 returned to baseline values at T6t. The blood gas analysis showed stability after hemorrhage and transfusion. Total protein was restored 1 hour after transfusion (T1t) and glucose at T6t. Albumin despite blood loss and transfusion remained unchanged in relation to T0. The renal variables, urea and creatinine, remained normal and constant and the GGT varied within the reference for the species. Thus, it can be concluded that the acute loss of 30% of blood volume is a recognized serious pathological event, causing clinical, hematological and biochemical changes in donkeys. However, GP donkeys were able to reverse changes in clinical and biochemical variables in up to 12 hours, with the exception of PT, and in up to 24 days in hematological variables, without transfusion. In turn, the GT animals had clinical, hematological and biochemical parameters reestablished within 6 hours and there was no liver or kidney damage after blood therapy, thus it is possible to conclude that homologous transfusion in donkeys is a safe and routine therapy. donkey clinic. | |
