Estenose aórtica grave: uma análise acerca das mudanças no tratamento nos últimos 10 anos

| dc.description.abstract | Introdução: A Estenose Aórtica (EAo) decorre da redução da área valvar efetiva por redução da mobilidade valvar. Indivíduos com estenose aórtica grave sintomática apresentam significativa redução da sobrevida, sendo mandatória a intervenção, mas a maioria desses pacientes são idosos com múltiplas comorbidades, tornando a cirurgia de troca valvar um procedimento de alto risco. Nas últimas décadas, o implante transcateter de valva aórtica (TAVI) surgiu como alternativa, principalmente nos pacientes com alto risco cirúrgico. Objetivos: Avaliar as mudanças no tratamento da EAo nos últimos 10 anos, identificando alterações nos parâmetros que norteiam decisões terapêuticas. Metodologia: Trata-se de uma revisão sistemática da literatura, foi utilizada a base de dados PubMed, tendo como descritores: “Transcatheter Aortic Valve Implantation”, “Severe Aortic Stenosis Treatment” e recorte temporal de 10 anos. Foram obtidos 221 resultados, sendo selecionados 23 documentos. Resultados: Durante o período observado, a TAVI foi consagrada como modalidade terapêutica alternativa à cirurgia. Diversos trials, como o PARTNER, evidenciaram sua eficácia e segurança, com aumento da classe de recomendação e nível de evidência. Discussão: A TAVI passou a ser escolha terapêutica para risco proibitivo e alternativa para riscos alto e intermediário e está, atualmente, em estudo para baixo risco, com resultados promissores. Essa técnica é menos invasiva que a cirurgia, apresentando incidências inferiores ou semelhantes de mortalidade e risco de AVC à cirurgia. Conclusão: A evolução dessa técnica possibilitou intervenções em pacientes antes inoperáveis, a redução da morbimortalidade e o aumento da expectativa de vida, podendo futuramente ser 1ª opção no tratamento da EAo. | |
| dc.description.abstract2 | Introduction: Aortic Stenosis (AS) results from the reduction of the effective valve area due to reduced valve mobility. Individuals with symptomatic severe aortic stenosis have a significant reduction in survival, and intervention is mandatory, but most of these patients are elderly with multiple comorbidities, making valve replacement surgery a high-risk procedure. In recent decades, transcatheter aortic valve implantation (TAVR) has emerged as an alternative, especially in patients with high surgical risk. Objectives: To evaluate the changes in the treatment of AoS in the last 10 years, identifying changes in the parameters that guide therapeutic decisions. Methodology: This is a systematic review of the literature, using the PubMed database, with the following descriptors: “Transcatheter Aortic Valve Implantation”, “Severe Aortic Stenosis Treatment” and time frame of 10 years. 221 results were obtained and 23 documents were selected. Results: During the observed period, TAVR was established as an alternative therapeutic modality to surgery. Several trials, such as PARTNER, have shown their efficacy and safety, with an increase in the class of recommendation and level of evidence. Discussion: TAVI became the therapeutic choice for prohibitive risk and an alternative for high and intermediate risks and is currently being studied for low risk, with promising results. This technique is less invasive than surgery, with lower or similar incidences of mortality and stroke risk compared to surgery. Conclusion: The evolution of this technique has allowed interventions in previously inoperable patients, reduced morbidity and mortality and increased life expectancy, which may in the future be the 1st option in the treatment of AoS. | |
