dc.description.abstractIntrodução: Os efeitos da covid-19 não estavam restritos somente aos sinais e sintomas provocados pela doença. Os esforços para evitar a propagação do vírus provocaram modificações no ambiente hospitalar, redução e modificação do atendimento médico, adiamento de procedimentos como exames de rastreamento e diagnósticos e procedimentos terapêuticos. Nesse contexto, a diminuição nos números de diagnóstico e tratamento pode contribuir para grandes incrementos nas taxas de morbimortalidade das enfermidades oncológicas, dentre elas o câncer colorretal, o terceiro câncer mais frequente no mundo e responsável por mais de dezoito mil mortes no Brasil anualmente. Objetivos: O presente artigo visa analisar o impacto da pandemia de COVID-19 nos dados de diagnóstico e tratamento do câncer colorretal. Além de caracterizar o perfil epidemiológico dos pacientes e as características dos serviços de saúde mais prejudicados pela pandemia, retratar as principais ações adotadas para o enfrentamento do câncer colorretal durante a pandemia de COVID-19, avaliar se a mortalidade foi maior que no período anterior à pandemia de COVID-19, avaliar se o diagnóstico foi retardado pelos efeitos da pandemia de COVID-19; analisar se os atrasos devido a pandemia resultaram em maior quantidade de diagnósticos de câncer colorretal em estágios mais avançados e fornecer dados para guiar possíveis soluções para o enfrentamento dos desafios que permeiam o câncer colorretal durante a pandemia de COVID-19. Metodologia: Foi elaborada uma revisão bibliográfica integrativa com 38 artigos pesquisados na plataforma PubMed com os seguintes descritores “ Neoplasic Colon rectal” AND “COVID-19”. Foram excluídos artigos em idiomas diferentes do inglês e português, não disponíveis por completo, indexados repetidamente e artigos que não entram nos critérios de inclusão. Revisão bibliográfica: Promover rastreamento de câncer colorretal diante do grande impacto devido ao início da pandemia de COVID-19 se tornou um desafio sobretudo devido à modificação dos fluxogramas desde o período pré-pandemico. Além disso, a promoção de lockdowns, a queda nos encaminhamentos, o medo de infecção por SARS-Cov-2 e remanejamento de exames, consultas e tratamentos. Grande esforços foram feitos para diminuir essa repercussão, dentre eles a incorporação da necessidade de obter segurança quanto ao bloqueio da transmissão do novo vírus como a opção por teleconsultas e o envio de testes de sangue oculto nas fezes (FIT) às residências dos pacientes, o rearranjo dos serviços à realidade imposta e o medo dos pacientes e profissionais da saúde quanto à enfermidade e ao futuro que lhes esperavam. No entanto, apesar das medidas houve um grande aumento de diagnósticos de adenomas de alto grau. Além disso, espera-se, em um futuro próximo, uma grande quantidade de diagnósticos de neoplasias em estágio mais avançado.
dc.description.abstract2Introduction: The effects of COVID-19 were not limited to the signs and symptoms of the disease itself. The efforts to avoid the spread of the virus modified the hospital environments, the shortening of medical appointments, the speeding up of procedures such as tracking exams and diagnostics and therapeutics. In this context, the decrease in diagnostics and treatment numbers may lead to large increases in morbimortality rates of oncology illnesses, among those colorectal cancer, the third most frequent worldwide, responsible for over eighteen thousand annual deaths in Brazil. Objectives: The presented paper aims to analyze the impacts of COVID-19 on diagnostics and treatment data for colorectal cancer. In addition to characterizing the epidemiological profile of patients and the characteristics of the health services most affected by the pandemic, portraying the main actions taken to face colorectal cancer during the COVID-19 pandemic, assessing whether mortality was higher than in the period prior to the outbreak; assessing whether diagnosis was delayed by the effects of the pandemic; analyzing whether pandemic-related delays have resulted in more colorectal cancer diagnoses at more advanced stages and providing data to guide possible solutions to the challenges that permeate colorectal cancer during the COVID-19 pandemic. Methodology: An integrative literature review was carried out with 38 articles searched on the PubMed platform with the following descriptors “Neoplasic Colon rectal” AND “COVID-19”. Articles in languages other than English and Portuguese, not fully available, repeatedly indexed and articles that do not meet the inclusion criteria were excluded. Literature Review: Promoting colorectal cancer screening in the face of the great impact due to the beginning of the COVID-19 pandemic has become a challenge, mainly due to the modification of flowcharts since the pre-pandemic period. In addition, the promotion of lockdowns, the drop in referrals, the fear of SARS-Cov-2 infection and the relocation of exams, consultations and treatments. Great efforts were made to reduce this repercussion, including the incorporation of the need for security in terms of blocking the transmission of the new virus with the option of remote consultations and the sending of fecal occult blood tests to patients' homes, the rearrangement of services to the imposed reality and the fear of patients and health professionals regarding the disease and the future that awaited them. However, despite the measures, there was a large increase in diagnoses of highgrade adenomas. Furthermore, it is expected, in the near future, a large number of diagnoses of neoplasms at a more advanced stage.
